The GBCHC provides primary medical care and pharmaceutical support for people who are:
- Uninsured
- 18-65 Years old
- A Michigan Resident
- at or below 250% Federal Poverty Level, click here to see if you fall within these guidelines
If you meet these criteria please go here to complete the needed paperwork.
Usually we are able to make an appointment within a few days of receiving all the needed paperwork.
If you have any further questions please contact the scheduling attendant at 248-309-3752. The best time to reach her is Monday, or Wednesday from 10am to 4pm.
