Diabetes Prevention Program Support

When and where

After completing the 16 Diabetes Prevention Program classes find continued support.

The first class will be held on Tuesday 3rd of November, 1:15pm- 2:15pm (Eastern Time)

Then the first and third Tuesday of every month

Prizes will be awarded to those that complete the program

Continue your diabetic education with the diabetes support classes

How to sign up

Those who have completed the Pre-Diabetes classes are automatically enrolled in the DPP support classes.

Members of the Life with Diabetes program are also welcome to attend for support and help putting healthy strategies to work.

Cost

Free to clinic patients.

A suggested donation of $10 per meeting (or optional $75 for one year program). Community scholarships are available. Healthy snacks and drinks provided.

Materials and content are created by the Centers for Disease Control. This program is funded by the CVS Caremark Charitable Trust and presented by clinic healthcare providers.
